Higher-Complexity ED Billing Codes — Sicker Patients, More Intensive Practice, or Improper Payments?

Between 2001 and 2010, billing for level 5 emergency department visits increased from 27% to 48% of Medicare discharges. Some blame up-coding facilitated by EHRs, but other factors are changes in demographics, practice patterns, and the emergency department's role.
