Presentation or CaseA 52-year-old man was admitted to the hospital because of chest pain.There was a history of insulin-dependent diabetes mellitus for 20 years. In addition, he had a long history of hypertension and a more recent finding of hyperlipoproteinemia. Nine years before entry the patient was first admitted to this hospital because of chest pain, and an electrocardiogram showed an acute anterior myocardial infarct with Q waves in Leads V1 and V2. He experienced angina during convalescence, with ST-segment depressions in Leads 2, 3, and aVF, and during one episode of chest pain there . . .
