IMPORTANT: This test will be graded by a machine. You must mark your answer firmly with a #2 pencil so that the machine can read it correctly. There are 50 questions.

The choices for “K” type questions are numbered 1, 2, 3, and 4. The answer key is always the same and is as follows:

A: 1, 2, and 3 are correct
B: only 1 and 3 are correct
C: only 2 and 4 are correct
D: only 4 is correct

If the choices are labeled A, B, C, and D, only one answer is correct. Mark your answers accordingly: A, B, C, or D.

  1. PRINT your name, last name first, in the boxes at the upper left of Side 1. Print your address above the name line and be certain to mark the letters of your name (last name first) in the columns beneath.

  2. In the “Identification Number” section, fill in your nine-digit customer number located on the mailing label above your address.

  3. Use only the upper right portion of Side 1; record your answers to questions 1-50. Do not make any marks on Side 2.

  4. Each question has only ONE correct answer. Use only circle 1, 2, 3, or 4 for corresponding answers A, B, C, or D. Do not use 5 (E).

  5. Fill in each circle completely using a #2 pencil. DO NOT USE INK.

  6. If you change an answer, erase the previous choice completely.

  7. Do not make any marks on the sheet except those described above.

  8. Keep a copy of your answer sheet for comparison with correct answers and critiques, which will be mailed with your score.

  9. Return answer sheet in enclosed envelope to:
