Bronchiectasis with Mycobacterium avium Complex Infection

Figure 1 An 81-year-old woman was admitted with weight loss (18 kg in 27 months), hemoptysis, and tubular and diffuse granular shadows on her chest radiograph (Panel A). Physical examination demonstrated persistent, coarse rales and diffuse rhonchi throughout the lungs, clubbing, and cyanosis. A high-resolution computed tomographic (CT) scan of the lungs (Panel B) demonstrated the presence of bronchiectasis, with ectatic markings; mucoid impaction; multiple small, well-circumscribed pulmonary nodules, some of which had cavities; and granular shadows throughout the lungs. A three-dimensional image of the lungs that was reconstructed from the CT scan showed saccular and varicose ectatic bronchi (Panel C).

Bronchoscopy was performed. Bronchial washings and sputum cultures yielded Mycobacterium avium complex. Bronchiectasis is commonly found in patients with M. avium complex infection.
