Figure 1 A 22-year-old man presented with a six-month history of severe epigastric and periumbilical hunger pangs, diarrhea, and a weight loss of 8 kg. Ultrasonographic abdominal examination showed normal findings, as did examination of a stool specimen for parasites, upper gastroduodenal endoscopy, total colonoscopy, and ileoscopy of the last 15 cm of the small bowel. On an x-ray film of the small bowel, the beef tapeworm Taenia saginata was detected as a long, translucent filling defect (arrows) in the preterminal ileal loop. The young man had been a regular consumer of raw beef prepared as steak tartare. Treatment with niclosamide (total dose, 2 g) resulted in both complete relief of abdominal symptoms within four days and a weight gain of 7 kg within four weeks. Three months after the treatment had been administered, a barium contrast study of the small intestine showed normal findings. The patient was followed for 18 months and remained asymptomatic.
