Article

Figure 1 Numerous flesh-colored umbilicated papules appeared on the face of a 33-year-old man with a three-year history of the acquired immunodeficiency syndrome (AIDS) and a CD4 cell count of 60 per cubic millimeter. Prominent lesions along the margin of the eyelids prevented him from closing his eyes completely (Panel A). Numerous similar lesions were also present in the groin, with scattered lesions on his scalp, buttocks, and legs. His systemic symptoms included diarrhea and fatigue. A biopsy revealed lobules of keratinocytes containing large eosinophilic intracytoplasmic inclusion bodies (Henderson–Patterson, or molluscum, bodies) (Panel B; hematoxylin and eosin, ×20). The patient had a poor response to treatment with liquid nitrogen, curettage, skin peeling with trichloroacetic acid, and tretinoin. There was some improvement approximately five weeks after the addition of indinavir to his systemic antiviral regimen, and his viral load decreased from 261,100 copies per milliliter to 193,500 copies per milliliter and the CD4 count increased from 30 cells per cubic millimeter to 75 cells per cubic millimeter. Systemically, his diarrhea resolved and he felt more energetic. Three months after starting indinavir treatment, his viral load increased to 293,900 copies per milliliter and his CD4 count decreased to 42 cells per cubic millimeter, although his clinical status remained stable.
