An asymptomatic 63-year-old man was found on routine physical examination to have palpable hepatic nodules and ballotable kidneys. Contrast-enhanced abdominal computed tomography showed many hepatic masses of various sizes (arrows, Panel A), whose measured density was consistent with that of water. Both kidneys (arrows, Panel B) also contained numerous cystic masses similar to those in the liver. The patient had microscopic hematuria and a serum creatinine concentration of 1.9 mg per deciliter (168 micromoles per liter). The results of other laboratory studies, including liver-function tests, were normal. The patient had no family history of liver or kidney disease, received no treatment, and remains well.
