A chest film was obtained in an 11-year-old girl who presented with pain and tenderness of the left iliac fossa. Chest auscultation revealed right-sided heart sounds. The position of the heart suggested situs inversus totalis. The left iliac fossa was incised to remove an acutely inflamed appendix. Other causes of left-sided appendicitis include situs inversus indeterminus (only the abdominal contents are transposed), a mobile cecum, and a very long appendix.
