To the Editor: In their article on methylnaltrexone for opioid-induced constipation in patients with advanced illness, Thomas et al. (May 29 issue)1 point out that the effectiveness of methylnaltrexone appeared to be undiminished throughout the 2-week, double-blind trial, as well as during the 3-month, open-label extension phase. However, during the trial, the percentage of positive responses to the drug decreased from 48% after the first dose to 38% after the dose received on day 13.
